- A physiatrist and physician specializing in physical medicine and rehabilitation play integral roles in your care team.
- Your care team also includes an admissions coordinator, nursing, case management, physical therapy, occupational therapy, speech therapy (if needed), wound care (if needed), dietician (if required), pharmacist, and Colleton Medical Center Physicians.
- A therapy program will be designed to help you achieve independence as soon as possible. The therapy team will focus on your limitations and provide at least three hours of therapy daily, 5-6 days per week.
- Team conferences occur weekly, focusing on patient progress, goals, and discharge planning. Following this meeting, we will inform you and your family of any concerns and prepare you for discharge needs/goals and plans.
- Patient/caregiver training occurs during your stay to ensure all needs are met and all care team members are prepared for discharge. Your entire team will be available for any questions/concerns you may have.
- We are Joint Commission Stroke Certified, meaning we specialize in caring for you or your family member after stroke. Our stroke program includes patient-centered care plans, daily education, a care binder that will follow you throughout your stay and post-discharge, an activity progression program for mobility safety, a weekly support group for you and/or your family during your admission and a monthly stroke support group post-discharge.
